POSITION REQUIREMENTS

ESSENTIAL FUNCTIONS:
1. Interprets and enters physicians orders into the computer system.
2. Maintains the patients computerized medication record with accurate entries into the system.
3. Monitors pharmacy patient profiles for patient allergies.
4. Monitors medication therapy for therapeutic duplications, contraindications and incompatibilities during the order entry process.
5. Monitors patients age 65 or older and high-risk patients for adequate renal function, making dosing change recommendations where appropriate.
6. Identifies appropriate dosages and routes of administration for patients in the following age-specific categories: Neonate/Infant, Pediatric, Adolescent, Adult, and Geriatric.
7. Under the supervision of the Clinical Pharmacist, monitors patients anticoagulation therapy by evaluating Creatinine Clearance when entering Enoxaparin into Meditech and the monitoring of excessive use of 10mg warfarin doses.
8. Under the supervision of the Clinical Pharmacist, monitors the dispensing of non-formulary medications, making the necessary changes as directed by current protocol.
9. Makes the appropriate IV to PO conversions of selected drug classes.
10. Provides pharmacokinetic dosing service for selected medications per physicians order, following current protocol and under the direction of the Clinical Pharmacist.
11. Participates in the reconciliation and refilling of anesthesia narcotic boxes or other special narcotic orders.
12. Utilizes the IV reconciliation process when preparing or monitoring the IV compounding process.
13. Prepares IV solutions, or supervises the technicians and interns in the preparation process.
14. Monitors TPN lab values with the assistance of the clinical pharmacist.
15. Monitors patients with apparent adverse drug reactions, participating in the reporting, corrective action, and outcome analysis of the drug reaction.
16. Supervises and verifies accuracy of technician drug distribution activities, correcting any discrepancies which may occur. Participates in the instruction of those technicians not meeting an acceptable standard.
17. Under the supervision of the Clinical Pharmacist, participates in the quality management, drug utilization evaluation, and performance improvement program in the pharmacy.
18. Helps monitor the monthly nursing station inspection, verifying that all areas are being inspected on a monthly basis.
19. Assists the buyer in stock procurement and stock control programs, ensuring that the appropriate drug is purchased to maximize cost containment programs.
20. Provides drug information to the professional staff, with no more than two incidences of inadequate or inaccurate drug information in any six-month period.
21. Attends department meetings as required. Completes mandatory in-services.
22. Projects a positive professional image along with a cooperative and pleasant attitude toward all members of the Hospital staff, patients, and fellow Pharmacy staff members, during working hours.
23. Completes the necessary continuing education to maintain a current Arizona Pharmacy license.
24. Participates in the hospital customer service program, using proper phone etiquette, following scripting, and maintaining patient confidentiality.
25. Participates in the facility awareness program by reporting any noticeable damage, by picking up trash and by promoting a professional image.
26. Adheres to the pharmacy policy and procedure relating to absences and tardiness. Provides proper notice of PTO requests.
27. Remains flexible with work schedule, as necessary, to meet department needs.
28. Maintains a working knowledge of, and complies with, all Hospital Policy and Procedures, and Pharmacy Policy and Procedure regulations not herein specified.


EDUCATION, EXPERIENCE & SKILLS

1. Bachelors Degree in Pharmacy or Pharm D
2. Minimum of one (1) year hospital pharmacy experience preferred
3. Strong Clinical Pharmacy skills preferred
4. Previous experience with Pharmacy computer systems preferred


LICENSURE / CERTIFICATION

1. Current AZ State Pharmacists License
